Doctor Todd Mitchell, who is responsible for counseling and treatment for 4 cases of poisoning, told NBC that he did not know the poisonous mushroom in the soup.

Many mushrooms in California state contain toxins. They can cause liver failure and death if the victim is not promptly treated. Amanita phalloides - the fungus that causes the most deaths - is shaped quite like a few fungi that humans can eat.
